A health intake form is completed at or before your first visit to our office. (These forms are available here on our web-site or we can fax, mail or e-mail them to you prior to your first visit.) It is imperative that these forms are filled out completely and accurately because they inform the doctors of your current complaint and your current health status.
You and one of our doctors will review your health intake forms together. A history of your health concerns will be reviewed, along with your current work and daily activity. You and one of our doctors will also investigate your current health status.
Clinical Examination & Evaluation
Your wellness evaluation will include a nervous system examination and a thorough spinal evaluation. These tests will help determine if you have lost your health or function in your body.
X-rays and Analysis
Your wellness evaluation may require x-rays. The x-rays reveal the degree of spinal misalignment, the phase of spinal degeneration, the presents or absence of significant pathologies, and potential contraindications of care. At Pinnacle Wellness, careful analysis leads to precision measurements. These measurements along with your clinical exam findings enable us to provide you with individualized treatment and precise and effective care.
